What are Dermal Fillers?

Dermal fillers are made of select ingredients that combine with our skin naturally. Fillers are used in many ways and the right filler for you will depend on your individual needs

What Dermal Fillers Do

1. Fillers restore lost volume and lift skin. When volume is restored to the mid face/cheeks the skin is given a lifting effect and thus you look more youthful and years can be reversed in just one treatment session.

2. Fillers refine skin texture and when injected in to the dermis the plumpness and hydration of the skin improves thus refining the skin texture. Minimizing pores and skin appearing brighter.

3. Fillers can also sculpt facial features. Areas like the nose, chin and jaw line are common areas treated for facial shaping but there are limitations and must be done by an experienced injector. Fillers do not replace plastic surgery and having worked for a facial plastic surgeon my knowledge and experience guide patients on filler versus surgery as their best option.

There is an endpoint with fillers – there can be too much placed and have you looking older or less natural. A variety of dermal fillers on the market can be used to individualize your results.

Allergan – Juvederm family of fillers and they are all hyaluronic acid based. Juvederm Ultra xc, Juvederm Ultra Plus XC, Voluma, Vollure, Volbella.

 

Galderma– Restylane family of fillers and Sculptra. Restylane, Restylane Silk, Restylane Lyft, Restylane Defyne, Restylane Refyne and Sculptra.

Merz– Belotero and Radiesse.

A more recent addition FDA approved is a hyaluronic acid filler by Prollenium called Revanesse Versa.

Bellafill is a Semi-permanent filler and is FDA approved to last up to 5 years but we do not offer this in our office.

How do I decide which filler to get?

Each filler has specific features, and these determine where they are placed and when they are used for best and safest outcome. For example, Voluma is placed in deeper tissues where as Volbella is placed superficially under the skin surface. Are dermal fillers safe and what side effects can I expect?

Yes. Dermal fillers have been proven to be very safe when injected properly and by trained healthcare professionals. Most common side effects (as listed by the FDA website at FDA.gov) are bruising, redness swelling, pain, tenderness, itching, rash and difficulty in performing activities (only observed when injected into the back of the hand). Less common side effects (as listed by the FDA website at FDA.gov) include: raised bumps in or under the skin (nodules or granulomas) that may need to be surgically removed, infection, open or draining wounds, a sore at injection site, allergic reaction, necrosis (tissue death).

Where can you use dermal fillers?

Most common areas injected are cheeks, smile lines, marionette lines, lips. Other areas treated include tear troughs, acne scars, ear lobes, back of the hand, jaw line, chin, nose and indentations from surgery or injury.

Are dermal fillers painful?

Most people tolerate the injections very well and describe the procedure as “pinchy” not painful but the level of discomfort is also dependent on the area being treated. Lips are a more sensitive area but we use a special numbing cream and most fillers have a numbing agent in the actual syringe to help with discomfort. On a pain scale of zero being no pain and 10 being the worst you have ever had most of my patients put it between a 1-3.

Will fillers replace the need for surgery?

No.  Some of the unhappiest filler patients are people who have come to me post injections with a different provider and are frustrated with the outcome being too insignificant. Unrealistic expectations of filler are frustrating, and you need to see a provider that knows the difference between needing a surgical procedure versus filler or other cosmetic procedures to achieve the results you are wanting. What I always tell my patients is that surgery is not for everyone but if you are informed on realistic expectations you will be happy with the outcome and will not feel surprised that you spent your hard-earned money and didn’t get what you thought you were getting.

Will I need to take time off work or have downtime?

I encourage patients to have the treatment done at a time where they can have a bruise and not be socially impacted. For example, have filler at least 2 weeks prior to any family photos, reunions, or work functions where you don’t want to answer why you are swollen or bruised. We use the smallest needle needed to help lessen the chance of bruising. It is a needle going through skin, so you may bruise. Some things you can do to lessen your chance of bruising – avoid taking any homeopathic supplements (like fish oil, vitamins, biotin, echinacea, St. Johns Wart, etc.). Medications like aspirin, Motrin, Aleve, and other NSAID medications and blood thinners are common to make people bruise more easily however Do not skip doses of medications prescribed to you. Eating fresh pineapple and taking Arnica Montana can lessen your bruising! For those who do not have downtime we have wonderful Glo Mineral Makeup and we can camouflage the bruising or we can do IPL to help lessen the appearance.

Do I have to stop my skincare?

Depending on the skin care brands you are using I like for patients to stop tretinoin (Retin-a), peels and no recent laser for at least 5 days prior. If you have recently had a deep peel or laser or if you are on a medication called Accutane it is a contraindication for filler. During the consultation we will go through every product you are using topically and all medications orally and we will make clear instructions customized to your plan of care for both before and after dermal fillers.

What if I want more or change my mind and want it “erased”?

I take pride in helping patients look and feel better and often that includes the advice of waiting before adding more. I guide my patients when to add more or stop. One of the great things about HA fillers is they can be dissolved by a medication.

How long will my results last?

he longevity of fillers is impacted by many variables such as product used, area injected, injection technique, medications patient is taking, lifestyle of heavy sun exposure or tanning bed use all impact the results lasting longer or shorter timeframes. Most HA fillers last 6 months up to 1 year. There are fillers with a longer FDA approval but for the most part if you expect 6-12months you will be happy with the longevity.

Can you combine fillers used?

I don’t combine different fillers in the same syringe, but it is very common to need more than one type of filler to achieve best results. For example, I love to do a lift and lips to many patients and Voluma is used for midface lifting and Volbella in the lips for an overall more youthful and rested look.
